Process for ordering court records works in Illinois

To order court records works in Illinois Department of Public Health you need to provide money orders, certified checks or personal checks. You should call or visit the World Wide Web to confirm the fees or for more information. You need to do the following for the birth certificates from January 1916.

You need to spend about dollar ten for certified computer abstract and fifteen dollars full certified copy of the original comments. Birth records in Illinois are not considered to be records for general public. The Access is restricted to the person named on the record. You can go for additional copies for two dollars each.

You need to include the following information:

a) The full name listed on the certificate
b) The date of birth
c) The city and country of birth
d) The full name of father
e) The full maiden name of mother
f) The reason for request
g) The relationship to the person named on the certificate
h) Your name, address
i) Your telephone number

For earlier records, you need to give written application to the country clerk in county.
