h) Your signature

For marriage records from July 1905 you need to pay seven dollars each. You need to include the following information with your request:

a) The full names on the marriage record
b) Date of marriage
c) Place of marriage
d) Your daytime telephone number with area code
e) Your mailing address
f) Reason for request
